Televisit Check-In
Welcome to the office of Dr. Michael Bublik Televisit Video Conference.
A televisit is a convenient way to have a live video conference with Dr. Bublik for a new patient evaluation or follow up visit.
If you are scheduled or need to schedule a televisit, please review carefully the following checklist:
STEP 1 – CALL TO SCHEDULE YOUR TELEVISIT
Please call: (818) 649-1433 or complete this online appointment form by choosing the televisit box to schedule your Televisit with Dr. Bublik. The office Staff will contact you to schedule your time.
STEP 2 – TECHNOLOGY AND INTERNET
Prior to your televisit, confirm you have a working smartphone, tablet, laptop, or desktop computer with a functioning camera, microphone, and speaker. You will need an adequate internet connection.
STEP 3 – FORMS NEEDED
Download, Print, and Complete the following forms prior to your televisit.
STEP 4 – CALL TO CHECK-IN
FIRST CALL, 818-649-1433 prior to your appointment time to check in with the receptionist and provide us pertinent information such as your forms.
STEP 5 – ENTER THE TELEVISIT WAITING ROOM
Click on the link below for Dr. Bublik’s Televisit waiting room at least 5 minutes before your scheduled appointment to be placed into the virtual waiting room.
Once Dr. Bublik is ready to meet with you, your televisit will start. We appreciate your patience during this process.
